I have just run Akeeba backup and will install it now
I am reading this:

I can;t get it full screen though so I am squinting a bit

OK, I am so close!
I have got the message button to appear, but I have to be logged in to see it.
If I could achieve what is in the pic attached as someone not logged in that would be perfect
Any ideas?

You can't send an anonymous private message. That will only function if the user viewing the profile is logged in.

I do get it, it makes sense how it works.
I am just concerned that people will miss the Email This User tab at the top when all the other menu items are under the profile pic etc.
People don't look for stuff if they don't have to.
So if I could get that menu item to be down with the other that would rock.
I am not sure if I can though?

The top menu bar is a navigation menu. The bottom are just tabs and they don't navigate the page anywhere. You can not combine the two. You are not missing anything as what you're wanting to do you just can't do. You can move the Menu tab into a different tab position if you don't like it at the top, but in the canvas layout it only has styling for it in that position so I've no idea how it'll display in other canvas positions.

I have moved the Menu button to the tabs and renamed it 'Menu & Email User' - I dunno, it looks OK but feels like a bit of a workaround
Would be awesome to somehow have the email form in a tab
