I've tried onCPayUserStateChange with no conditions.
In that case it should trigger on all transactions, right?
I logged out and created a new account... The action wasn't executed.
I'm really wondering why...
It seems it did fire when creating an account with another browser... I may just have a cache / cookies issue. Is that possible?
I'll put the conditions back in and see what happens.

It depends, is the plan a subscription plan or a merchandise plan. I don't really know what to tell you. Most of the integrations are using that trigger. If it's not firing then either plugins are not being loaded in whatever situation your site is in at the time or there's something wrong with the usage. You really should be creating new CB Plugins for PHP usages. CB Auto Actions CAN do PHP usages, but I highly recommend not doing so unless it's an absolute last resort; you should create a new CBSubs plugin (review existing CBSubs plugins) to specifically and correctly do what you need.

It's a merchandise... But now that you make me think about it maybe I should try to make this with a subscription.
If I felt capable of creating a CBSubs plugin properly I'd have already tried... But I've never even made just a Joomla plugin.
