Your issue is caused by the 3rd party CB plugin "Radius Search for Community Builder". It has unclosed HTML tag somewhere in it causing the HTML on the page to break. This is a pretty serious problem as it can cause all kinds of unforeseen issues and have disabled it for you within CB > Plugin Management. I've confirmed your issue is now resolved. Please contact the developer of "Radius Search for Community Builder" and have them fix their plugin.

Remove the double quote from that closing tag. I can't really advise you on anyones coding but our own. That plugin isn't extending the search criteria properly either as it should just be a new fieldtype plugin to add a custom radius field.
